The NHS has strict rules:
✔ You must be 18+ with:
A BMI over 40 (or over 37.5 if you’re from a Black, Asian or minority ethnic background)
At least 4 serious weight-related health conditions (like diabetes, heart disease or severe sleep apnoea)
Please note:
This is not for general weight loss
Our team cannot prescribe it unless you meet all these requirements
Asking if you don’t qualify takes time away from patients who need urgent care

Once you turn 50, the NHS sends you a free, at-home bowel screening test. It’s quick, easy, and can catch cancer early—even before symptoms appear. Don’t wait; do your test and return it. If you notice any symptoms, contact your GP right away.
Bowel cancer is more treatable when caught early!

Living with heart failure can be challenging, but with the right support, you can stay in control of your health. The Brent Community Heart Failure Service is here to help you manage your condition and recognise when you need extra support.
Understanding Your Symptoms
Knowing the signs of worsening heart failure helps you take action at the right time. Here’s what to look out for:
When to Contact Us (Amber Signs – Call for Advice)
Your weight increases by 2kg (4lbs) or more in 3 days AND you feel more breathless than usual.
Your legs or ankles are more swollen than normal.
You feel breathless at night or need extra pillows to sleep.
You’re less active than usual or feeling more tired or muddled.
If you notice these changes, call our Patient Initiated Follow-Up (PIFU) service for advice before your symptoms get worse.
When to Seek Urgent Help (Red Signs – Act Now)
Severe breathlessness or chest pain.
Fast, fluttering, or pounding heartbeat.
Fainting, dizziness, or blackouts.
Worsening confusion or other medical problems.
Diarrhoea or vomiting for more than 24 hours.
If you experience any of these, do not wait—contact your GP, call 111, or dial 999 immediately.

Help you understand your condition and medications.
Monitor your weight, blood pressure, and overall health.
Adjust your treatment if needed to keep your symptoms under control.
